           7      Q.   Please state your full name.
           8      A.   Lynnette Doan-wiggins.
           9      Q.   And your professional address, 133 N,
         10   probably?
         11       A.   Yes, Loyola University Medical  Center,  2160
         12   South First Avenue, Maywood, Illinois.
         13       Q.   Would you please tell me about  your  medical
         14   education?
         15       A.   University of Cincinnati  Medical  School.  I
         16   completed a residency in emergency medicine  there,  a
         17   fellowship with an emphasis on re8earch there.
         18       Q.   And from there, where did you go?
         19       A.   University Hospital, Jacksonville,  which  is
         20   an affiliate of the University of Florida.  I was
         21   there for about two years, nine months, then to the
         22   University of Chicago where I was an  attending  there
         23   for approximately four years.
