           7  BY MR.  DEVER:
           8         Dr. Stockfish, for the record, would you give
           9         us your full name and your business address?
         10   A.     Yes.  My first name is Hyman, H-y-m-a-n.
         11          middle initial M, Stockfish is the last name;
         12          3609 Park East, Suite 418 North,  Beachwood,
         13          Ohio, 44122..
         14   Q.     And you are a physician by practice, is that
         15          right --
         16   A.     Yes, Sir.
         17   0.     -- profession?
         18              And you do not have -- I  don't  believe
         19          you have a written CV, a curriculum vitae?
         20   A.     I did.
         21   0.     You don't have it now?
         22   A.     I couldn't find it.
         23   Q.     okay.  I won't try to go too far back, but
         24          starting with your undergraduate, would  you
         25          give me a resume of your education?

          1   A.     I went to Ohio State from 1949 to 1953, and
          2          received a Bachelor of Science; I went to Ohio
          3          State Medical School from 1953 till 1957,  and
          4          received my M.D. degree; I interned at  the
          5          Mt. Sinai Medical Center in Cleveland  from
          6          July lst of 1957 to June 30th, 1958; I was  a
          7          Captain of the United States Air Force from
          8          August of 1958 through August of 1960.
          9              My first year of residency at the
         10          Mt. Sinai Medical Center was from January  lot
         11          of 1961 through December 31st of '61; my
         12          second year of residency was at the old  Crile
         13          VA Hospital on the west side of  Cleveland,
         14          from January Ist of 162 through December  31st
         15          of '62; and my third year as senior  resident
         16          at the Mt.  Sinai Medical Center, from  January
         17          lst of '63 through December 31st of '63.
